While our secure intake system is being finalized, the online coverage-check form does not transmit or store the information you enter; when it goes live, it will run only through HIPAA-conscious systems.
• Technical information: standard server logs (IP address, browser type, pages viewed). Analytics tools are currently disabled on this site; if enabled in the future, they will be configured so that form contents and other health-related details are never transmitted to analytics or advertising platforms.

How We Use Information

We use inquiry information for one purpose: responding to you and helping you plan care. We do not sell inquiry information, share it with advertisers, or use it for unrelated marketing.
